At any time, a participating professional may be eliminated from the respective network in which he/she is associated. 

Outlook Health, L.L.C. in providing benefits and discounts in this program is not a licensed insurer, health maintenance organization, or other underwriter of health care services.  No portion of any provider’s fees will be reimbursed or otherwise paid.  The discounts contained herein may not be used in conjunction with any other discount plan or program.  All listed or quoted prices are current prices from participating providers and subject to change without notice.  From time to time, certain providers may offer products and/or services to the general public at prices lower than the discounted prices available through this program.  In such event, members will be charged the lowest price. 

Providers are subject to change without notice and programs may vary in some states.  This is a discount membership program only, not insurance, and may be discontinued or modified at anytime.  This program is a referral and discount plan and does not warrant professional services, nor is it responsible for the quality of care received.  This program makes no warranties expressed or implied concerning services or care provided. 

Member is defined as a primary member, spouse, and all legal dependents.  This contract is not covered by any life or health guarantee association. 

Though we make every effort to preserve your privacy we may need to disclose personal information when required by law or in the good-faith belief that such action is necessary in order to conform to the edicts of the law or comply with a legal process served on our Web site.

With the purchase of the Outlook Prescription Plan, the Outlook Dental Plan, or the Outlook Complete Plan at an annual rate, each enrollee agrees that unless you are otherwise advised in advance by Outlook Health, L.L.C., all electronic payment methods (credit card or ACH), the Plan membership will be automatically renewed at the expiration of the initial annual enrollment and the same method of payment will be charged for renewal.  Email confirmation will be provided upon renewal of membership with Outlook Health, L.L.C.

Often times, other Web sites refer their users to the Outlook Health or subsidiary Web sites. These sites often pass a non-personally identifiable digit to the Outlook Health Web site.

If you cancel for any reason within 30 days of the enrollment date, you will receive a full refund.  Non-refundable one-time fees will be disclosed at the time of application.
